Case Management Practice - Introductory Level

This workshop aims to assist practitioners at the introductory level to understand the frameworks and models that underpin case management practice and its processes including how to engage individuals and/or families in their case planning and goal setting.

  • Demonstrate an understanding of case management practice and models

  • Outline the legal requirements of case management within the community services sector 

  • Ethical practice in case management 

  • The role and responsibilities of a case manager in various work contexts

  • Complete purposeful documentation 

  • Develop meaningful case plans & goal setting with clients 

  • Work collaboratively with clients and other services 

  • To actively engage clients in the change process

  • To effectively utilize supervision and reflective practice

  • Develop and implement a worker’s wellbeing & self-care plan
